Mal-PEG12-DOTA reacts with the targeted molecules (such as antibody fragments, peptides) that have been pre-reduced to free -SH groups at the Mal end to generate Target-Mal-PEG12-DOTA. Then, the DOTA end is incubated with radioactive metal isotopes (such as ⁶⁸GaCl₃) in a weakly acidic or near-neutral buffer. DOTA quantitatively chelates the metal, resulting in the final Target-PEG12-DOTA-⁶⁸Ga reagent.
