It recognizes small molecules with a cyclized tetrahydroisoquinoline, such as non-steroidal neuromuscular blocking drugs like tubocurarine and atracurium. Upon binding, MRGPRX2 mediates pseudo-allergic reactions marked by histamine release, inflammation, and airway contraction. Additionally, it serves as a receptor for various ligands, including peptides (cortistatin-14, proadrenomedullin N-terminal peptides PAMP-12 and PAMP-20, antibacterial protein LL-37, PMX-53 peptide, and beta-defensins) and alkaloids like complanadine A.
