BRAF Mutation Colorectal Cancer

BRAF-mutated colorectal cancer (CRC) is a distinct molecular subtype characterized by specific clinicopathological features, occurring in approximately 10%-15% of cases, with a higher prevalence in right-sided colon cancers. The most common mutation is V600E, leading to constitutive activation of the BRAF protein and dysregulation of the MAPK signaling pathway, which controls cell growth, proliferation, and differentiation. This results in uncontrolled cell growth and tumorigenesis. BRAF mutations are somatic and not inherited, acting as oncogenic drivers in CRC and other cancers such as melanoma and thyroid papillary carcinoma. These mutations serve as both prognostic markers—associated with poorer outcomes—and predictive biomarkers, particularly indicating resistance to anti-EGFR therapies and guiding the use of targeted treatments in metastatic disease. Current clinical management includes specific BRAF-targeted therapies for metastatic BRAF-mutated CRC, underscoring the importance of molecular testing in treatment decision-making.
