BCMA-Targeted CAR-T for MM

B-cell maturation antigen is a highly relevant surface target on malignant plasma cells, making BCMA-directed CAR-T therapy a major advance for relapsed or refractory multiple myeloma. BCMA supports plasma-cell survival through ligands such as APRIL and BAFF and is more restricted to the B-cell lineage than many alternative targets. Autologous BCMA CAR-T products can induce deep responses after multiple prior therapies, including in disease resistant to proteasome inhibitors, immunomodulatory drugs, and antibodies. Relapse remains common, so research has shifted from initial response toward durability, sequencing, and salvage. The marrow microenvironment, antigen evolution, and quality of manufactured T cells all influence long-term control Clinical development followed earlier evidence from BCMA-directed antibodies and established the antigen as both a therapeutic handle and a marker of plasma-cell biology[1][2][3].

The CAR binds BCMA independently of MHC and activates CD3ζ plus costimulatory signaling, leading to plasma-cell killing, cytokine release, expansion, and potential memory formation. Lymphodepletion supports post-infusion expansion. Resistance can arise through low BCMA density, antigen downregulation or loss, soluble BCMA, selection of resistant clones, impaired death pathways, or insufficient CAR persistence. Repeated stimulation and inhibitory marrow signals promote exhaustion, while stromal and myeloid cells provide survival support to residual myeloma. A deep clinical response can therefore coexist with small resistant populations that later repopulate marrow or extramedullary sites CAR architecture and manufacturing influence expansion and persistence, so two BCMA products cannot be assumed to share identical kinetics or resistance profiles[1][3].

Clinical use requires collection, bridging treatment, lymphodepletion, infusion, and monitoring for cytokine release syndrome, neurotoxicity, cytopenia, infection, and hypogammaglobulinemia. Delayed neurologic and hematologic effects require product-specific follow-up. At relapse, reassessing BCMA and disease distribution guides salvage. BCMA-retained disease may respond to another BCMA modality, but prior exposure, interval, and T-cell fitness matter. Antigen-low disease motivates targets such as GPRC5D, dual-target CARs, bispecific antibodies, or non-cellular therapy. Earlier use may capture fitter T cells, yet it must be compared with effective standard combinations and account for manufacturing access Because normal plasma cells also express BCMA, effective therapy predictably impairs humoral immunity and may require vaccination planning, prophylaxis, or immunoglobulin replacement[1][2][3].

Future development should prioritize prevention and classification of relapse. Dual-target recognition, optimized costimulation, memory-preserving manufacturing, allogeneic platforms, and armored cells may improve durability. Serial marrow, blood, and extramedullary sampling should measure antigen density, soluble BCMA, CAR kinetics, clonality, and immune state. Minimal residual disease can refine response assessment but must be linked to sustained progression-free and overall survival. Trials should define sequencing among CAR-T cells, bispecific antibodies, antibody-drug conjugates, and transplantation. BCMA CAR-T therapy has transformed late-line myeloma, but its next gains will depend on maintaining immune surveillance and selecting timely, mechanism-matched therapy when the tumor escapes Real-world studies should include older, frail, renally impaired, and extramedullary-disease populations who may be underrepresented in registration trials yet frequently need treatment. Resistance-guided salvage should be planned before infusion so that target testing, referral, and an alternative therapy are available at progression. Access improvements must reduce manufacturing failure and treatment delay without weakening rigorous long-term patient safety surveillance[1][2][3].

Linvoseltamab (REGN5458) is a bispecific T-cell engager (BiTE) antibody that specifically binds to B cell maturation antigen (BCMA) and CD3 of T cells, thereby directing T cells to multiple myeloma (MM) cells expressing BCMA and activating T cells to kill tumor cells. Linvoseltamab can be used in research of relapsed/refractory multiple myeloma (RRMM).
