Epigenetic differences between a tumor margin and its core refer to spatial variation in DNA methylation, chromatin accessibility, histone regulation, and related transcriptional states across regions of the same lesion. The margin experiences direct contact with surrounding stroma, vasculature, extracellular matrix, and immune cells, whereas the core more often contains hypoxic, nutrient-limited, and mechanically stressed niches. A measured difference may reflect a stable cancer-cell program, a transient response to local conditions, or simply a different proportion of nonmalignant cells. The topic should therefore be treated as a spatial heterogeneity problem rather than as a universal binary in which every tumor edge and core carry the same fixed marks[1][2].

Multi-region profiling demonstrates why single biopsies can misrepresent this landscape. Hepatocellular carcinomas show marked methylation heterogeneity among spatially separated regions even when their genomes are comparatively stable, indicating that epigenetic diversification can proceed beyond mutational branching. Epigenetic field effects also extend into histologically nonmalignant tissue near prostate tumors, where quantitative methylation measurements vary with distance from the lesion. These findings support a continuum from tumor-adjacent tissue through invasive fronts to internal subclones. A core-biased sample may overrepresent stress-adapted or poorly perfused cells, while a margin-biased sample may capture invasive programs, stromal contamination, or immune interactions. Single-cell DNA methylation, single-cell ATAC sequencing, and multimodal assays can separate cell-intrinsic chromatin states from changes caused by different mixtures of cancer, immune, endothelial, and fibroblast populations. Spatial multi-omics can then retain tissue coordinates and connect molecular states to histology, immune exclusion, invasion, and treatment penetration. Sampling matched regions before and after therapy could identify whether resistance pre-existed in a protected niche or emerged through treatment-induced reprogramming. This framework could reveal region-specific vulnerabilities and reduce false conclusions caused by averaging opposing subclones[2][4].

The central research gap is the absence of common spatial definitions, matched sampling schemes, and sufficiently large longitudinal cohorts. Pathologists and sequencing studies do not always define the invasive margin, peritumoral stroma, viable core, necrotic core, and tumor nests consistently. Many platforms also measure only one epigenetic layer or lose spatial information during tissue dissociation. Future studies should analyze matched regions with controlled tumor purity, integrate methylome, chromatin accessibility, transcriptome, and proteome data, and validate inferred regulators through perturbation. Computational deconvolution should be checked against imaging or cell-resolved measurements rather than assumed to recover the true tissue architecture. Until such studies mature, margin-core claims should report tumor type, anatomical region, sampling distance, cell composition, and assay resolution rather than generalize a spatial signature across cancers Prospective studies should preregister regional boundaries and analytical endpoints so spatial signatures can be compared across centers instead of remaining cohort-specific observations[1][2][4].
