NF-κB Fuels Inflammation, Autoimmunity, Cancer

NF-κB is an inducible transcription factor system first defined through κ immunoglobulin enhancer binding and later linked to post-translational activation, IκB-mediated cytoplasmic inhibition, inflammation, autoimmunity, and cancer[1][2]. The field remains central to life sciences because NF-κB regulates innate and adaptive immune functions, inflammatory gene expression, cell survival, and tumor-promoting inflammatory circuits[3][4][5]. Dysregulated NF-κB activation contributes to chronic inflammatory disorders, autoimmune pathology, and inflammation-associated tumor development, making it a core framework for studying immune regulation, cytokine signaling, tumor microenvironment biology, and therapeutic resistance[4][6][7][8].

NF-κB signaling operates through tightly controlled canonical and non-canonical pathways[3][5]. In the canonical pathway, inflammatory cytokines and immune stimuli activate the IκB kinase system, which promotes IκB phosphorylation, ubiquitination, proteasomal degradation, NF-κB nuclear translocation, and transcriptional activation of inflammatory and survival genes[2][3][4]. In the non-canonical pathway, selected TNF receptor superfamily signals stabilize NIK, activate IKKα, induce p100 processing to p52, and generate RelB-p52 transcriptional activity[5]. These pathways induce cytokines, chemokines, inflammasome-related programs, leukocyte activation, inflammatory T-cell differentiation, and immune-cell survival, while defects or excess activation disturb immune tolerance and drive autoimmune inflammation[4][5][6].

Disease applications follow directly from this biology. In inflammatory and autoimmune disease research, NF-κB functions as a drug-discovery target because deregulated activation is associated with chronic inflammatory disease, while NF-κB also supports central tolerance and regulatory T-cell function[4][6]. In cancer biology, NF-κB links persistent infection and chronic inflammation to increased cancer risk, tumor-elicited inflammation, anticancer immunity, tumor-cell survival, proliferation, angiogenesis, metastasis, metabolism, and therapy resistance[7][8][9]. In colitis-associated cancer, IKKβ deletion in intestinal epithelial cells reduced tumor incidence without reducing inflammation, while IKKβ deletion in myeloid cells reduced expression of proinflammatory cytokines that promote tumor growth[8].

Key gaps center on therapeutic precision rather than target validation. Broad NF-κB blockade risks suppressing host defense, immune surveillance, and tissue homeostasis because the same pathway mediates inflammatory pathology and protective immunity[4][6][7]. Future research should prioritize cell-type-specific NF-κB targeting, pathway-selective IKKβ or NIK modulation, temporal control of inflammatory transcription, and combination strategies that separate tumor-promoting inflammation from antitumor immunity[5][7][9]. The most useful clinical direction is not global NF-κB inhibition, but context-specific modulation of canonical and non-canonical signaling in inflammatory disease, autoimmunity, and cancer[4][5][6][9].
