Cancer vaccines and immunotherapy

Therapeutic cancer vaccines aim to generate or amplify immune responses against antigens expressed by an established tumor. Platforms include peptides, proteins, nucleic acids, viral vectors, dendritic cells, and tumor-derived material. Earlier vaccines often targeted shared tumor-associated antigens, which can be limited by immune tolerance and normal-tissue expression. Genomic sequencing enabled personalized neoantigen vaccines directed at mutations absent from healthy cells, while mRNA technology made rapid, multivalent manufacturing more practical. Vaccines are generally better at priming and broadening T-cell repertoires than at reversing every suppressive feature of advanced tumors, so current development emphasizes rational combinations and treatment of low-burden or minimal residual disease The distinction between prevention and therapy is important: an established cancer has already undergone immune editing and built mechanisms that resist newly primed cells[1][2][3].

An effective vaccine must deliver antigen to professional antigen-presenting cells together with danger or adjuvant signals. A multivalent vaccine can address tumor heterogeneity, but subclonal or poorly expressed mutations may contribute little. Manufacturing time matters in aggressive disease, and patient-specific products require coordinated sequencing, bioinformatics, release testing, and clinical scheduling. Toxicity is often less acute than with cell therapy, yet autoimmunity, inflammatory reactions, and off-target antigen selection remain relevant Clinical development must also distinguish failure to prime from failure of primed cells to enter or recognize the tumor, since the corrective intervention differs[1][3].

Key gaps include reliable epitope prioritization, proof of tumor trafficking, and determination of the best disease stage. Immune responses measured in blood do not guarantee recognition of tumor cells, especially when antigen presentation is defective. Trials should connect vaccine-induced clonotypes with tumor infiltration, target-cell killing, and recurrence outcomes. Shared benchmarks for prediction pipelines and prospective validation can reduce false candidates. Improved lymph-node delivery, self-amplifying or circular RNA formats, and coordinated CD4 and CD8 antigen design may increase potency. Cancer vaccines are most likely to succeed as part of a system that combines precise antigen selection, effective dendritic-cell activation, relief of local inhibition, and enough time for adaptive immunity to mature Durable memory, breadth against multiple clones, and compatibility with standard therapy may ultimately matter more than the magnitude of one early blood response.
