Ufmylation: Mechanisms, Cellular Functions, And Role In Disease

UFMylation is a ubiquitin-like post-translational modification in which ubiquitin-fold modifier 1 (UFM1) is covalently conjugated to protein substrates through an enzymatic cascade. The pathway has rapidly become a major research area because it maintains endoplasmic reticulum homeostasis, protein biogenesis, cellular differentiation, DNA damage response, proteostasis, autophagy, and cancer-associated signaling. Its biological importance is reinforced by disease studies showing that dysfunction of UFMylation contributes to neurological disorders, liver disease, metabolic dysfunction, hematological disorders, skeletal dysplasias, cardiovascular diseases, and cancer[1][2].

Mechanistically, UFMylation uses UBA5 as the E1-like activating enzyme, UFC1 as the E2-like conjugating enzyme, and UFL1 as the E3 ligase. Structural work shows that UBA5 transfers UFM1 to UFC1 through a short linear sequence and that a region outside the UBA5 adenylation domain is critical for UFM1 transfer. UFL1 binds UFC1 through an N-terminal helix, competes with UBA5 for UFC1 binding, and coordinates E1-E2-E3-mediated UFM1 transfer to substrates. These studies define the UFM1 conjugation machinery as a tractable regulatory system and provide a basis for developing UFMylation inhibitors[3][4][5].

Cellular functions converge on ER-linked protein quality control, ribosome-associated quality control, autophagy-lysosome regulation, and genome stability. Ribosomal UFMylation is associated with quality control at the ER, and RPL26 is confirmed as a UFMylated substrate in liver models. CDK5RAP3 and RPL26 UFMylation regulate ribosome dissociation, while hepatocyte differentiation and liver homeostasis depend on this ribosomal UFMylation safeguard. DDRGK1 loss promotes autophagy induction by impairing mTOR signaling but blocks autophagic degradation by inhibiting autophagosome-lysosome fusion, suppressing lysosomal function, and aggravating apoptosis. UFMylation also regulates early ribosomal DNA damage response, because UFL1 and DDRGK1 localize to nucleolar caps after rDNA damage and UFL1 loss impairs ATM activation and rDNA transcriptional silencing[6][7][8].

Disease applications now include neurodevelopmental disease, fatty liver disease, obesity-associated metabolic dysfunction, liver injury with Mallory-Denk body formation, cancer biology, and drug discovery. Biallelic UFM1 and UFC1 mutations cause severe early-onset encephalopathy with progressive microcephaly and widespread reduction of cellular UFMylation, supporting an essential role in human brain development. In liver disease, UFMylation molecules are downregulated in human alcoholic and nonalcoholic steatohepatitis and in DDC-refed mice where Mallory-Denk bodies form. In metabolism, UFMylation-deficient DDRGK1 reduces FASN stability, adipocyte lipogenesis, body weight, fat mass, and improves glucose homeostasis in high-fat diet mice. The main research gaps remain substrate identification, tissue-specific disease mechanisms, tumor microenvironment biology, and therapeutic selectivity, making UFM1, UBA5, UFC1, UFL1, DDRGK1, RPL26, and FASN central targets for future translational studies[1][2][6][9][10][11].
