Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD)

Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly grouped under nonalcoholic fatty liver disease, identifies hepatic steatosis in a metabolic-risk context. Its increasing prevalence parallels overnutrition, physical inactivity, obesity, insulin resistance, and metabolic syndrome. MASLD can progress from steatosis to metabolic dysfunction-associated steatohepatitis, fibrosis, cirrhosis, and hepatocellular carcinoma, while also increasing type 2 diabetes, chronic kidney disease, cancer, and cardiovascular risk. Cardiac-related causes account for much of the mortality burden, so MASLD is now studied as a cardiovascular-liver-metabolic disorder rather than an isolated hepatic phenotype. The nomenclature shift has intensified work on mechanisms, risk classification, and treatment-response heterogeneity Its multisystem definition is clinically important because cardiovascular events can occur without parallel progression of fibrosis, and severe liver disease can develop in a lower-risk metabolic phenotype[1][2].

Hepatic lipid accumulation produces lipotoxicity that engages endoplasmic-reticulum stress, oxidative stress, organelle dysfunction, ferroptosis, and inflammatory injury. Unhealthy diet, dysfunctional adipose tissue, insulin resistance, and subclinical inflammation are shared upstream drivers of liver and cardiovascular disease. The steatotic, inflamed liver can then alter systemic physiology through excess glucose production, atherogenic dyslipidemia, procoagulant factors, hepatokines, and microRNAs, affecting endothelium, myocardium, and vasculature. Gut microbial dysbiosis may further modify lipid handling and inflammatory signaling. These reciprocal pathways help explain associations with atherogenesis, diastolic dysfunction, valvular calcification, arrhythmia, and epicardial fat, but the relative contribution of liver-specific injury versus common metabolic exposure varies among patients Hepatic inflammation also changes coagulation and vascular signaling, so mechanistic studies should quantify liver-derived mediators rather than attribute every association to shared obesity or diabetes[1][3][4].

Clinical application begins with dual risk assessment. Large population data associate MASLD and related steatotic liver disease with myocardial infarction, ischemic stroke, heart failure, and cardiovascular death. Noninvasive serum and imaging tests can identify hepatic disease, while cardiovascular screening should not depend solely on established atherosclerotic risk factors. Weight loss remains the most effective nonpharmacological intervention, but liver and cardiometabolic responses to treatment are not uniform. Data-driven clustering has identified a liver-specific phenotype with genetically linked, rapidly progressive liver disease and limited cardiovascular risk, and a cardiometabolic phenotype marked by dysglycemia, hypertriglyceridemia, and higher cardiovascular and diabetes risk. This distinction supports tailored surveillance and metabolism-based therapy instead of treating identical liver imaging as a uniform prognosis The cluster findings provide a practical hypothesis for precision care, but simple clinical variables must reproduce these groups prospectively before they guide treatment intensity[2][4][5][6].

Major gaps concern causality, phenotyping, and endpoint selection. Current data do not yet justify routine precision medicine, and improvement in steatosis, fibrosis markers, or body weight does not guarantee cardiovascular benefit. Resmetirom and incretin-based approaches primarily address metabolic liver injury, whereas hepatic inflammation remains a key driver of progression from steatosis to steatohepatitis and fibrosis. Future trials should stratify liver-specific and cardiometabolic phenotypes, measure hepatic and cardiovascular outcomes together, and define how genetics, adipose dysfunction, microbiota, immune state, fibrosis, and treatment exposure modify response. Multidisciplinary care must also determine when cardiologists should screen for MASLD and when hepatologists should intensify cardiovascular prevention. The field needs validated biomarkers that distinguish shared metabolic risk from liver-derived cardiovascular signaling and identify patients who require pathway-specific intervention Outcome studies should report liver failure, diabetes, kidney disease, cardiovascular events, cancer, and mortality together to avoid optimizing one organ while leaving another risk unchanged[1][5][6][7].
