Cellular Senescence and Cardiovascular Aging

Cellular senescence is a durable cell-cycle arrest induced by telomere attrition, DNA damage, oxidative stress, metabolic dysfunction, oncogene signaling, or epigenetic change. Senescent cells remain metabolically active and can release a senescence-associated secretory phenotype containing inflammatory, proteolytic, and growth-modifying factors. In the cardiovascular system, senescence affects endothelial cells, smooth-muscle cells, cardiomyocytes, fibroblasts, valve interstitial cells, and immune populations. These cells accumulate with age and at sites of atherosclerosis, myocardial infarction, fibrosis, valvular disease, and heart failure. Research now treats senescence as a heterogeneous tissue state that can drive vascular dysfunction and remodeling, rather than simply a marker of old chronological age Senescence can be adaptive when it stops damaged-cell proliferation or organizes short-term repair, but persistent survival and secretory activity make chronic accumulation harmful[1][2].

Endothelium is particularly exposed to hemodynamic forces and circulating damage. Tumor-suppressor pathways enforce arrest, while altered morphology and secretory activity impair vasodilation, barrier regulation, angiogenesis, and inflammatory balance. Across the vascular wall, senescence interacts with mitochondrial dysfunction, genomic instability, loss of proteostasis, deregulated nutrient sensing, stem-cell dysfunction, and chronic low-grade inflammation. Paracrine factors can spread dysfunction to neighboring cells and distant organs, linking local arterial aging to systemic metabolic disease. Mitochondrial energy failure and reactive oxygen species can both initiate and sustain the senescent phenotype. Therapeutic approaches include senolytics that selectively kill vulnerable senescent cells, senomorphics that suppress harmful secretory programs, and interventions that reduce upstream stress. Dasatinib, quercetin, fisetin, and navitoclax emerged by targeting anti-apoptotic networks that allow senescent cells to survive their own secretions. Intermittent dosing is proposed because cells reaccumulate slowly, and preclinical models show benefits across multiple age-related disorders. In experimental atherosclerosis, single-cell and spatial transcriptomics identify senescence-enriched smooth-muscle, fibroblast, and T-cell clusters and a vascular signature involving SPP1, CTSB, and TNFRSF11B; senolytic treatment reduces these populations. These tools may support disease tracking and target selection, but cardiovascular outcome evidence in humans remains insufficient Spatial signatures can distinguish senescent cells within fibrous caps, media, adventitia, or myocardium and relate their removal to local structural consequences[3][4][5][6].

The major gap is identification. No single marker proves senescence across all cardiovascular cell types, and p16 expression, DNA-damage signals, secretory factors, or mitochondrial dysfunction can appear in other states. Removing beneficial transient senescence could impair wound healing, tumor suppression, or tissue remodeling, whereas indiscriminate senolysis may damage nonsenescent cells that use the same survival pathways. Future trials should establish tissue or circulating biomarker panels, define which cell population is causal, and measure vascular function, plaque stability, fibrosis, heart failure, frailty, and adverse effects together. Longitudinal human studies should separate biological age from disease burden and determine optimal timing. Precision geroscience will require senescent-cell subtype, anatomical niche, and secretory phenotype to guide whether prevention, reprogramming, or selective elimination is appropriate Clinical trials should verify both depletion of the intended senescent population and preservation of wound healing, hematopoiesis, immune defense, and normal tissue turnover[1][5][6][7].
