CAR-T Therapy Challenges: Tumor Relapse

Tumor relapse is the principal limit on the curative potential of CAR-T therapy. A patient may never achieve a deep response, recur soon after initial clearance, or relapse after a prolonged remission, and those patterns can reflect different biology. Resistance is commonly divided into antigen-positive relapse, in which target remains but CAR-T cells are insufficient, and antigen-negative relapse, in which tumor recognition has been lost. This distinction emerged prominently with CD19-directed therapy and now guides post-relapse biopsy and treatment selection. Modern research treats relapse as an evolving interaction among tumor genotype and phenotype, CAR design, manufactured T-cell quality, host immunity, and the tissue microenvironment rather than as a single defect Relapse timing can provide an initial clue, because immediate progression and recurrence after long cellular persistence rarely have identical causes[1][2][3].

Antigen-negative escape can arise through mutation, altered splicing, reduced surface density, lineage change, or selection of a pre-existing antigen-low clone. Trogocytosis may transfer target molecules to CAR-T cells, simultaneously lowering tumor antigen and provoking fratricide or exhaustion. Antigen-positive relapse instead points toward limited expansion, poor persistence, tonic receptor signaling, terminal differentiation, checkpoint inhibition, defective cytotoxic signaling, or resistance of tumor cells to apoptosis. High tumor burden and inflammatory stress can accelerate dysfunction. In solid tumors, trafficking and metabolic suppression add further barriers; in marrow and lymphoid tissue, stromal and myeloid signals also shape persistence. The same patient can harbor more than one mechanism, so a binary antigen test is necessary but not sufficient Host anti-CAR immunity can add another layer by accelerating clearance of cells that express immunogenic receptor components[1][2].

Clinical management should reassess target expression, disease distribution, tempo, and prior CAR kinetics whenever tissue is safely obtainable. If the original antigen persists, options may include a second cell product, a different construct, bispecific antibody therapy, targeted drugs, or transplantation in selected settings. Antigen-negative disease requires a new target such as CD20 or CD22 rather than simple re-exposure to the same receptor. Multispecific CARs, sequential targeting, and tandem receptors aim to prevent escape before it occurs. Improving manufacturing from less differentiated T cells, adjusting costimulation, reducing tonic signaling, or adding checkpoint resistance may address cell-intrinsic failure. Rescue choices must also consider prolonged cytopenia, infection risk, and the patient’s ability to undergo another intensive therapy Rapid access to validated antigen testing is therefore part of treatment infrastructure, not merely a research convenience after failure[1][3].

The major research gap is the lack of standardized, longitudinal relapse profiling. Trials should pair baseline tumor material with samples after infusion and at progression, while measuring antigen density, CAR expansion, phenotype, clonality, cytokines, and tumor death pathways. Circulating tumor DNA may reveal emerging relapse earlier, but tissue remains important for identifying antigen and lineage. Preventive engineering should be matched to observed risk: dual targeting for heterogeneous antigen, memory-preserving production for poor persistence, and microenvironmental combinations for localized suppression. More potent cells are not automatically more durable or safer. The goal is a controllable immune population that maintains surveillance as tumor cells adapt, supported by a relapse taxonomy that directs rational salvage instead of empiric sequencing Shared definitions of antigen loss, persistence failure, and lineage change would also make resistance studies more comparable across products[1][2][3].
