Cardiovascular Aging: Characteristics, Pathways, and Therapeutic Targets

Cardiovascular aging is the progressive loss of vascular and cardiac resilience that occurs through cumulative molecular damage, altered repair, and lifelong exposure to hemodynamic and metabolic stress. It is characterized by endothelial dysfunction, arterial stiffening, impaired microvascular regulation, myocardial hypertrophy and fibrosis, reduced metabolic flexibility, and greater vulnerability to ischemia and heart failure. Mechanistically, oxidative stress, mitochondrial dysfunction, chronic inflammation, genomic instability, cellular senescence, epigenetic change, loss of proteostasis, deregulated nutrient sensing, and stem-cell exhaustion interact across vessels and myocardium. Age is therefore not only a time variable but a biological context that changes cellular response to conventional cardiovascular risk factors Cardiac and vascular aging interact: a stiff arterial system raises ventricular workload, while reduced cardiac reserve worsens organ perfusion and accelerates systemic vulnerability[1][2].

Vascular aging alters both structure and function. Smooth-muscle phenotype change, medial calcification, fibrosis, and extracellular-matrix remodeling increase stiffness; loss of Windkessel function raises pulse pressure and contributes to isolated systolic hypertension, microvascular damage, heart failure, vascular cognitive impairment, retinopathy, and kidney disease. In the aging heart, substrate use shifts, mitochondrial oxidative capacity declines, reactive oxygen species rise, and autophagy or mitophagy becomes less effective. Senescent cells and inflammatory signals reinforce fibrosis and hypertrophy. Therapeutic targets span risk factors and geroscience mechanisms. Blood-pressure, lipid, glucose, smoking, and activity control remain foundational, while exercise, caloric restriction, and metabolic drugs may improve nutrient sensing and mitochondrial function. Senolytics or senomorphics aim to reduce harmful senescent-cell effects; anti-inflammatory strategies address inflammaging; and mitochondrial, autophagy, epigenetic, or stem-cell interventions seek to restore resilience. Arterial stiffness can provide an intermediate phenotype for prevention and treatment studies. Translation requires separating normal adaptation from treatable pathology. Older adults differ in frailty, multimorbidity, medication burden, biological age, and the dominant cardiac or vascular phenotype, so chronological age cannot select one therapy. Biomarkers should integrate arterial stiffness, endothelial function, metabolic state, senescence, inflammation, and organ function and demonstrate responsiveness to intervention. Trials need outcomes important to older adults, including mobility, cognition, independence, hospitalization, and adverse drug effects alongside cardiovascular events. Cell and animal models should incorporate age rather than extrapolate from young systems. The field must also establish whether targeting one hallmark yields durable benefit or triggers compensation through another. Precision cardiovascular geroscience should identify the limiting resilience mechanism in each patient and test safe restoration rather than pursue nonspecific anti-aging treatment Prevention studies should begin before advanced fibrosis or calcification while still including older adults with established disease to determine the limits of reversibility[1][5][6][7].
