Autophagy And TFEB In TB Immunity

Autophagy is a lysosome-dependent pathway that can restrict intracellular Mycobacterium tuberculosis while maintaining macrophage homeostasis. Cargo recognition, autophagosome formation, fusion with lysosomes, acidification, and degradation together constitute autophagic flux. In infected macrophages, xenophagy and related phagosome-maturation processes can deliver bacilli or bacterial material to degradative compartments. TFEB coordinates lysosomal biogenesis and expression of autophagy genes, making it a central transcriptional regulator of this defense. The pathway also interacts with lipid metabolism, mitochondrial quality, endoplasmic-reticulum stress, and inflammatory signaling, all of which influence whether macrophages control bacilli or develop damaging inflammation[1][2][3].

M. tuberculosis actively remodels this system. The pathogen induces miR-33 and miR-33-star, which repress ATG5, ATG12, LC3B, LAMP1, fatty-acid oxidation, and upstream AMPK-FOXO3-TFEB activity, supporting lipid-body accumulation and intracellular persistence. Conversely, silencing these miRNAs enhances lipid catabolism and xenophagy in experimental models. TRIM27 provides another regulatory route: after infection, nuclear TRIM27 can cooperate with CREB1 at the TFEB promoter, increase autophagy-related gene expression, and reduce intracellular bacterial survival independently of its E3-ligase activity. A TFEB activator rescued defects caused by TRIM27 loss in cellular and mouse systems, supporting causal pathway organization. Autophagy is widely used in normal tissues, so systemic activation can have metabolic, immune, and toxic effects; TB-associated inflammation also differs by stage and lesion. TFEB is a promising nodal target, but safe benefit depends on restoring balanced lysosomal, mitochondrial, and inflammatory function. Better bacterial killing without control of neutrophil-driven tissue damage may not improve lung function, while reducing inflammation without sterilization may favor persistence[1][3][4][5].

Mitochondrial function is also coupled to antimicrobial autophagy. SIRT3 supports respiratory homeostasis and promotes PPAR-alpha and TFEB-related programs. SIRT3 deficiency in experimental infection increased mitochondrial oxidative stress and excessive inflammation while impairing host defense; restoration of PPAR-alpha or TFEB improved antimicrobial activity. These findings suggest that autophagy failure is not simply too little degradation but part of a metabolic-inflammatory state. However, measurements of LC3 puncta or autophagosome abundance can be misleading because accumulation may reflect increased formation or blocked clearance. Studies must measure flux, lysosomal function, bacterial colony-forming units, cell viability, and inflammatory injury together. Host-directed therapy could activate TFEB, AMPK, SIRT3, PPAR-alpha, vitamin D receptor pathways, or selected autophagy machinery, or inhibit pathogen-induced miR-33 circuits. Such treatment would complement, not replace, multidrug antimicrobial therapy, and must not create a nutrient or survival advantage for infected cells. Drug-resistant and drug-sensitive organisms could respond differently because bacterial stress and antibiotic exposure modify host-pathogen interactions[1][2][3][5].

Candidate agents should be tested with standard antibiotics in human-relevant macrophages, granuloma models, and well-designed clinical trials. Pharmacodynamic evidence of enhanced flux and reduced viable bacilli is required, together with monitoring for immunopathology. The desired immune effect also needs precise definition. Human genetic variation, diabetes, malnutrition, HIV status, and concomitant medicines may alter autophagy or lysosomal capacity. Trials should therefore stratify relevant host factors and include sputum conversion, relapse, lesion resolution, lung function, and toxicity. Biomarkers from peripheral blood should be validated against pulmonary compartments because circulating cells may not reflect macrophages within granulomas or cavities. Pulmonary validation remains indispensable[1][3][5].
