Tumor-Infiltrating Lymphocyte (TIL) Therapy

Tumor-infiltrating lymphocyte therapy expands a patient’s own lymphocytes from a resected tumor and returns them after lymphodepleting conditioning. The platform preserves a naturally selected, polyclonal repertoire that may recognize multiple shared and private tumor antigens. Early melanoma studies established durable regressions with TIL-based adoptive therapy, and pooled clinical evidence for lifileucel in advanced melanoma after checkpoint and targeted therapy renewed the field. Unlike CAR-T therapy, TILs do not require one predefined surface antigen; unlike a single TCR product, they can cover multiple peptide–MHC targets. Their advantages are balanced by variable starting material, individualized manufacturing, intensive conditioning, and dependence on intact antigen presentation Regulatory progress has converted decades of individualized academic experience into a more standardized product model, while retaining the biological complexity of each patient’s tumor[1][2][3].

TILs are isolated from tumor fragments, expanded under cytokine support, and selected or enriched for growth and tumor reactivity. Lymphodepletion reduces competing and suppressive cells and increases homeostatic cytokines before infusion, while interleukin-2 can support expansion afterward. Effective clones must traffic back to lesions, recognize peptide–MHC, form cytotoxic synapses, and persist. The harvested population can include bystander, terminally exhausted, and tumor-reactive cells, so cell composition matters. Chronic stimulation, inhibitory receptors, metabolic stress, and loss of tumor antigen presentation can limit function even when large numbers of cells are infused Culture duration is a tradeoff because expansion supplies dose but can reduce clonal diversity, proliferative reserve, and the tissue-homing properties needed after infusion[1][2].

The most established application is advanced melanoma, particularly after progression on immune checkpoint inhibitors, but programs are expanding into cervical, lung, ovarian, head and neck, and other solid tumors. Surgical access to a suitable lesion is required, and manufacturing time can be difficult for rapidly progressive disease. Toxicity arises from lymphodepleting chemotherapy, cytokine support, infection risk, cytopenia, and capillary or organ stress rather than from a synthetic receptor alone. New approaches select neoantigen-reactive clones, use less differentiated cells, add checkpoint resistance, or shorten culture. Centralized production must preserve product identity while accommodating biological variability Response assessment should account for the interval required for manufacturing and conditioning so that outcomes are not attributed only to the infused product[1][3].

Future trials should connect T-cell clonotypes and phenotypes in the infused product with trafficking, persistence, antigen recognition, and clinical response. Rapid assays are needed to distinguish tumor-reactive cells from bystanders before expansion. Nonmyeloablative conditioning and cytokine support should be optimized to reduce toxicity without sacrificing engraftment. Combination with checkpoint blockade or vaccines may sustain useful clones, but timing must avoid redundant or excessive inflammation. TIL therapy demonstrates that a patient’s tumor already contains therapeutic information; the next advance is to capture the most relevant cells, manufacture them reproducibly, and maintain their function after they re-enter the suppressive tissue from which they were isolated A scalable future may combine centralized quality systems with local tumor procurement and rapid digital transfer of manufacturing and immune-monitoring data. Broader adoption will depend on predictable vein-to-vein time, transparent product specifications, and conditioning regimens that centers can deliver safely outside a few expert programs[1][2][3].
