Cellular Senescence & Age-related Diseases

Cellular senescence is a stable state of cell-cycle arrest triggered by diverse forms of cellular stress, including telomere shortening, DNA damage, oncogene activation, mitochondrial dysfunction, oxidative stress, and chromatin perturbation. Originally described as replicative senescence in cultured human fibroblasts, senescence is now recognized as a fundamental biological process involved in tissue remodeling, tumor suppression, wound healing, aging, and age-related diseases. Senescent cells remain metabolically active and acquire characteristic phenotypic changes, including altered chromatin organization and secretion of bioactive molecules. Accumulation of senescent cells increases with age in multiple tissues and has been linked to functional decline and chronic disease. Consequently, cellular senescence has become a major research focus in geroscience and translational medicine because it represents a potential mechanistic driver of aging and a therapeutic target for age-associated disorders[1][2][3][4].
The molecular regulation of senescence is controlled primarily through the p53-p21 and p16INK4a-RB tumor suppressor pathways. DNA damage responses activate ATM, ATR, CHK1, and CHK2 signaling, leading to stabilization of p53 and induction of p21-mediated cell-cycle arrest. In parallel, activation of p16INK4a reinforces retinoblastoma protein-dependent suppression of cell-cycle progression. Senescent cells also develop a senescence-associated secretory phenotype (SASP), characterized by secretion of inflammatory cytokines, chemokines, growth factors, and proteases. The SASP modifies tissue microenvironments, influences immune surveillance, alters stem-cell function, and contributes to chronic inflammation. While transient senescence can facilitate tissue repair and suppress malignant transformation, persistent senescent-cell accumulation promotes age-related pathology through sustained inflammatory signaling, extracellular matrix remodeling, and disruption of tissue homeostasis. These dual functions place senescence at the intersection of tumor suppression, inflammation, and organismal aging[2][3][5][6].
Cellular senescence has been implicated in numerous age-related diseases, including neurodegenerative disorders, cardiovascular disease, osteoarthritis, pulmonary fibrosis, metabolic dysfunction, diabetes, and cancer. Experimental studies demonstrated that genetic or pharmacological elimination of senescent cells can improve tissue function and delay multiple age-associated phenotypes in animal models. These findings stimulated development of senolytic agents that selectively eliminate senescent cells and senomorphic approaches that suppress SASP activity without inducing cell death. Despite significant progress, important challenges remain. Senescent cells are heterogeneous across tissues and disease states, no universal biomarker exists for their identification, and beneficial versus detrimental senescence programs are incompletely understood. Future research should focus on defining senescence subtypes, improving biomarker-guided patient stratification, characterizing senescence-associated immune interactions, and developing precise interventions that preserve physiological functions while reducing pathological senescent-cell burden in age-related diseases[4][5][7][8].
