Acute Pancreatitis: Pathophysiology And Inflammatory Networks

Acute pancreatitis begins with injury to pancreatic acinar cells and can progress from a self-limited local illness to systemic inflammation and multiorgan failure. Gallstones, alcohol, hypertriglyceridemia, procedures, drugs, and genetic susceptibility are major causes. Within acinar cells, abnormal calcium signaling, mitochondrial dysfunction, co-localization of zymogens with lysosomal hydrolases, premature digestive-enzyme activation, impaired autophagic flux, and pathological secretion damage cellular membranes. Necrotic, apoptotic, and regulated inflammatory forms of cell death release enzymes and danger signals into the interstitium, where they activate resident and recruited immune cells[1][2].

Local injury expands through an inflammatory network. Acinar, ductal, endothelial, neural, and immune cells produce cytokines, chemokines, lipid mediators, reactive oxygen species, and proteases. Microcirculatory failure and oxidative stress increase ischemia and necrosis. MIF can signal through CD74/CD44 and CXCR receptors and interact with Toll-like receptor and inflammasome pathways, making it a potential amplifier and biomarker. When mediator release exceeds local containment, endothelial activation, capillary leak, coagulopathy, lung injury, kidney injury, and shock develop. Pro- and anti-inflammatory signals overlap, and later immune dysfunction can increase susceptibility to infected necrosis or sepsis[1][3][4].

The intestine is both a target and amplifier. Hypoperfusion and systemic inflammation impair epithelial integrity, while dysmotility and altered nutrition affect microbiota. Gut-derived cytokines and bacterial translocation can aggravate pancreatic and distant-organ injury. Early enteral nutrition supports epithelial restitution and is generally preferable when tolerated, but claims for probiotics or microbial clearance require attention to disease severity and safety; results from one formulation cannot be generalized. The timing of interventions is critical because the initial acinar events occur before many patients present, whereas later complications may be driven by necrosis, infection, or organ failure. Current management is primarily supportive and cause directed: early severity assessment, appropriate fluid resuscitation, analgesia, enteral nutrition, treatment of biliary obstruction when indicated, organ support, and staged management of necrosis or infection. Numerous anti-cytokine approaches have succeeded experimentally but failed to establish reproducible clinical benefit, reflecting narrow therapeutic windows, model differences, and patient heterogeneity. Trials must stratify etiology and phase and measure organ failure, infection, intervention burden, mortality, and long-term pancreatic function, not only circulating cytokines[1][2][3][4][5].

New therapies targeting calcium overload, autophagy, MIF, inflammasomes, cell death, extracellular vesicles, or delivery platforms should be paired with mechanistic biomarkers and standard care. Biomarkers and predictive models should support, not delay, bedside management. Hemoconcentration, organ dysfunction, systemic inflammatory measures, imaging, and clinical course can inform severity, but no single early score perfectly predicts progression. Fluid therapy must avoid both inadequate perfusion and overload, particularly in cardiac or renal disease. Prophylactic antibiotics are not justified for sterile necrosis, whereas documented infection requires appropriate treatment and source control. Gallstone pancreatitis, hypertriglyceridemic disease, and alcohol-associated disease need prevention of recurrence after recovery. Follow-up should address exocrine insufficiency, diabetes, nutrition, pain, and quality of life. These clinical endpoints ensure that mechanism-based intervention improves the whole disease trajectory rather than one acute laboratory value. Etiology-specific prevention and multidisciplinary follow-up are therefore integral components of successful treatment, even after the initial inflammatory episode has resolved. across clinical settings[1][3][4].
