PANoptosis and Cardiovascular Diseases

PANoptosis is an inflammatory lytic cell-death program in which pyroptotic, apoptotic, and necroptotic machinery is coordinated within a PANoptosome. It is initiated by innate immune sensors and executed through interacting caspases and receptor-interacting protein kinases. This integration matters because inhibiting only one conventional death pathway may not stop the combined program; key sensor or PANoptosome components can remain able to recruit alternative execution machinery. Cardiovascular research now implicates PANoptosis in ischemia-reperfusion injury, cardiomyopathy, atherosclerosis, myocardial infarction, myocarditis, and aortic disease, extending a concept developed largely in infection, inflammation, and cancer into cardiovascular pathology The concept therefore describes coordinated pathway crosstalk and should not be used merely as a collective synonym for several independent forms of death[1][2].

Cardiomyocyte ischemia-reperfusion provides direct mechanistic evidence. ZBP1 rises during injury and assembles a ZBP1-RIPK3-CASP8-CASP6 complex that drives noncanonical PANoptosis; cardiomyocyte-specific ZBP1 deficiency reduces death and infarct size more effectively than conventional single-pathway inhibitors in experimental models. Mitochondrial integrity supplies another control point in doxorubicin cardiotoxicity. FUNDC1 interacts with TUFM, limits cytosolic mitochondrial-DNA release, and restrains PANoptosome activation, whereas FUNDC1 deficiency worsens mitochondrial injury, cardiac dysfunction, and PANoptosis. These studies connect nucleic-acid sensing, mitochondrial damage, innate immunity, and mixed death execution within the same cardiac cell These mitochondrial and sensor-driven mechanisms explain why interventions upstream of execution may outperform combinations of late pathway inhibitors[1][3][4].

Additional sensors broaden therapeutic possibilities. IFI16 detects viral or aberrant DNA and can participate in pyroptotic, apoptotic, and necroptotic signaling relevant to heart disease. The mechanosensitive channel PIEZO1 interacts with caspase-8 during hypoxia-reoxygenation; pharmacological inhibition reduces infarct size, oxidative stress, inflammation, and PANoptosis-related mediators in experimental ischemia-reperfusion. In doxorubicin injury, RND3 restrains ROCK1-dependent phosphorylation of DRP1, limits mitochondrial fission, and reduces the combined death response. Candidate applications therefore include reperfusion injury, anthracycline cardiotoxicity, heart failure, diabetic cardiomyopathy, and inflammatory vascular disease, but most evidence remains preclinical Mechanical stress, aberrant DNA sensing, and mitochondrial fragmentation provide distinct initiating contexts that can converge on similar lytic outcomes[2][4][5][6].

The central gap is diagnostic specificity. Simultaneous detection of markers from three pathways does not by itself prove an integrated PANoptosome or distinguish coordinated death from mixed cell populations dying separately. Future studies need cell-resolved temporal measurements, biochemical demonstration of complexes, genetic perturbation of shared nodes, and comparison with pathway-selective inhibition. Human cardiac tissue and circulating biomarkers must be linked to clinical outcomes and disease stage. Therapeutic programs should prioritize upstream sensors or scaffold interactions while testing infection risk, immune consequences, and tissue specificity. Because PANoptosis can involve different complexes under different stimuli, one universal inhibitor is unlikely; mechanism-defined patient and injury subgroups will be essential Validated pharmacodynamic markers will be necessary to show that an experimental inhibitor actually disrupts PANoptosome activity in the target tissue. Genetic epistasis and biochemical complex isolation should accompany marker panels to demonstrate that pyroptotic, apoptotic, and necroptotic components operate within a coordinated mechanism. Human studies must also identify the responsible cell type and initiating sensor before pathway-directed treatment. Serial sampling should establish whether complex assembly precedes irreversible myocardial loss[1][5][6][7].
