Disulfide Stress and Cardiovascular Diseases

Disulfide stress is a specialized redox imbalance in which cellular thiol-control systems fail and abnormal disulfide bonds or oxidized cysteine species accumulate. It can arise when glutaredoxin and thioredoxin pathways are overwhelmed or disrupted, producing persistent protein glutathionylation, intermolecular disulfides, and irreversible thiol oxidation. Unlike a generic increase in reactive oxygen species, the concept emphasizes cysteine chemistry, protein conformation, and the enzymes that write or reverse disulfide bonds. Cardiovascular research links this stress to atherosclerosis, hypertension, myocardial ischemia-reperfusion injury, diabetic cardiomyopathy, hypertrophy, and heart failure because contractile, metabolic, endothelial, and inflammatory proteins depend on precisely regulated thiol states The defining event is not oxidation itself but failure of regulated thiol exchange, which changes protein folding, enzyme activity, adhesion, and signaling in a site-specific manner[1][2].

Disulfide formation can be damaging or protective according to the protein and compartment. In ischemic cardiomyocytes, FMO2 acts as a chaperone that catalyzes disulfide formation in unfolded or misfolded proteins through its GVSG motif, reduces the unfolded-protein response, and limits CHOP- and caspase-12-associated apoptosis; deleting FMO2 worsens survival, whereas cardiomyocyte overexpression protects infarcted hearts in experimental systems. In pulmonary vascular cells, an oxidant-induced disulfide between cyclin D and CDK4 inhibits kinase activity, lowers retinoblastoma-protein phosphorylation, and arrests proliferation. Pulmonary arterial hypertension shows reduced CDK4 disulfide formation, while auranofin restores this redox brake and reduces experimental vascular remodeling FMO2-mediated folding and inhibitory kinase disulfides show that adding or removing a disulfide can be protective depending on the protein and cellular compartment involved[1][2][3].

Redox switches also control vascular barrier function and thrombosis. TNIK phosphorylates ezrin-radixin-moesin proteins to drive TNF-dependent endothelial contraction and permeability, but hydrogen-peroxide-mediated oxidation of TNIK Cys202 forms an inhibitory intermolecular disulfide. Integrins couple mechanical force to redox state at focal adhesions; disulfide reduction in platelet integrin αIIbβ3 by ERp5 under shear helps regulate adhesion, making thiol chemistry relevant to thromboinflammation. These pathways suggest therapeutic opportunities in ischemia, pulmonary hypertension, edema, diabetes-associated vascular inflammation, and platelet activation. Research must distinguish regulated disulfide switches, proteotoxic accumulation, and disulfidptosis-like cell death. Static oxidation measurements do not show whether a cysteine modification is causal, reversible, compartmentalized, or merely a consequence of injury. Future studies should use redox proteomics, site-directed genetics, structural assays, live-cell sensors, microfluidic shear systems, and dynamic force measurements to map when specific disulfides form and how they change protein function. Mitochondrial dynamics and mitophagy should be measured because mitochondrial dysfunction increases reactive oxygen species and intersects with several programmed-death pathways and macrophage states. Therapeutic design should favor protein-, cysteine-, tissue-, and timing-specific modulation while monitoring immunity and normal redox adaptation. The most credible targets will be validated switches whose correction restores cardiovascular function without broadly suppressing oxidation Mapping reversible cysteine states during injury and recovery could identify intervention windows before abnormal bonds become irreversible protein damage or trigger downstream inflammatory cell death[1][4][5][6].
