Acute Pancreatitis: Mechanisms And Treatment Methods

Acute pancreatitis is a sudden inflammatory injury of the pancreas whose clinical course ranges from self-limited disease to necrosis, sepsis, and persistent organ failure. Gallstones, alcohol, and hypertriglyceridemia are leading causes, with regional variation. Injury begins within acinar and ductal compartments but becomes a systemic disease when inflammatory mediators, vascular dysfunction, and metabolic products affect distant organs. Aberrant digestive-enzyme activation remains important, yet current models also include pathological calcium entry, mitochondrial permeability transition, impaired autophagy, endoplasmic-reticulum stress, and regulated or necrotic cell death. The interaction among these processes, rather than one lesion, determines severity[1][2][3].

Calcium overload and mitochondrial failure can deplete ATP and promote acinar-cell necrosis, while defective autophagy and an overwhelmed unfolded-protein response impair cellular protection. Injured cells and infiltrating leukocytes then generate cytokines and danger signals that amplify local damage. Current management emphasizes early diagnosis, severity assessment, supportive care, appropriate fluid resuscitation, analgesia, nutrition, and management of the cause. Classification systems separate mild, moderately severe, and severe disease by local complications and organ failure. Clinical trials have changed nutritional support and the management of infected necrosis, favoring carefully timed and often step-up interventions. However, there is no broadly effective molecular therapy that reliably prevents severe progression. Proposed approaches target calcium entry, mitochondrial injury, inflammation, cell-death pathways, extracellular vesicles, and other mechanisms, while biomarker and multicytokine panels may enrich future trials for high-risk patients. Excessive early anti-inflammatory treatment could impair later control of infected necrosis, so stage-specific intervention is critical. Core outcome sets would further strengthen comparison[1][2][3][4][5].

Research should focus on the early therapeutic window before self-sustaining systemic injury is established. Predictive models must be externally validated, rapidly available, and shown to improve decisions rather than merely forecast outcome. Trials should stratify by etiology, time from symptom onset, necrosis, infection, and organ-failure phenotype and use clinically meaningful endpoints. Organoid, ex vivo, and animal models should reproduce both pancreatic injury and systemic complications. The most promising path is to link a measurable causal mechanism to a time-limited therapy in an enriched population while continuing evidence-based supportive and interventional care Etiology-specific treatment remains important: biliary obstruction, severe hypertriglyceridemia, alcohol-associated disease, medication-related injury, and post-procedural pancreatitis have different preventable or reversible components. Fluid strategy must balance perfusion with overload, and invasive treatment of necrosis should be guided by infection, symptoms, and clinical trajectory rather than imaging alone. Molecular trials should be embedded in this modern care framework; otherwise an apparent drug effect may reflect unequal supportive management[1][2][3][4][5].
