Aging-Associated Chronic Inflammation

Aging-associated chronic inflammation, commonly termed inflammaging, is a persistent, sterile, low-grade inflammatory state that develops in many older adults. Elevated inflammatory markers correlate with multimorbidity, frailty, disability, and premature mortality, and inflammaging is now integrated with the major biological hallmarks of aging. The concept does not imply that every older person has the same inflammatory program. Rather, genetic susceptibility, lifetime exposures, body composition, infections, and organ-specific damage combine to produce heterogeneous trajectories. Cardiovascular disease provides strong evidence for clinical relevance, while associations also extend to diabetes, chronic kidney disease, cancer, depression, dementia, and sarcopenia; whether direct anti-inflammatory intervention improves all of these outcomes remains unsettled[1][2].

Multiple sources feed the inflammatory state. Endogenous debris, microbial products, nutrients, and misplaced self-molecules activate a limited repertoire of innate sensors. Central adiposity, increased intestinal permeability, microbiome changes, chronic infection, NLRP3 inflammasome activation, mitochondrial oxidative stress, and immune-cell dysregulation add overlapping inputs. Senescent cells release a senescence-associated secretory phenotype that can propagate inflammation and induce senescence in neighboring cells. In parallel, aged immune cells clear senescent cells and inflammatory mediators less efficiently, creating a self-reinforcing loop. T-cell aging contributes through thymic involution, mitochondrial and genomic changes, loss of proteostasis, repertoire contraction, naive–memory imbalance, senescence, and reduced effector plasticity. Single-cell and spatial technologies can identify which cells and organs sustain inflammation and clarify whether the same pathway drives frailty, vascular disease, and neurodegeneration[1][3][4][5].

Inflammaging also interacts bidirectionally with other hallmarks of aging. Disabled autophagy, dysbiosis, altered nutrient sensing, mitochondrial dysfunction, and proteostatic failure promote inflammatory signaling; persistent inflammation then worsens each of these defects. Metaflammation driven by nutrient excess shares mechanisms with inflammaging, helping explain the convergence of metabolic disease and biological aging. Proposed interventions include physical activity and dietary modification, control of cardiometabolic risk, senolytic or senomorphic strategies, modulation of inflammasome and cytokine pathways, and approaches directed at microbiota or immune metabolism. However, inflammatory signaling also supports host defense and tissue repair, so reduction of a biomarker cannot be assumed to produce healthier aging. Studies must also account for medications, latent infection, obesity, and comorbidity, which can change inflammatory measurements independently of intrinsic aging. Repeated measurements are essential because an acute infection can resemble inflammaging in a single sample, whereas the target phenotype is persistent and systemically coordinated[1][2][3][4][5].

The field now needs causal, longitudinal, and stratified evidence. Biomarker panels based on methylation, glycomics, metabolomics, lipidomics, cellular phenotypes, and inflammatory proteins may distinguish biological from chronological age, but they require standardization and validation across populations. Trials should measure resilience, physical and cognitive function, vaccine response, infection susceptibility, and multimorbidity rather than one inflammatory marker alone, and should determine whether benefit varies by baseline inflammatory endotype. The central translational question is whether early, selective interruption of self-amplifying inflammation can delay several age-related diseases without increasing infection or impairing repair. Answering it will require combining geroscience biomarkers with mechanism-specific interventions and long-term clinical outcomes Diversity in ancestry, socioeconomic exposure, diet, and pathogen burden must be represented so that an apparent aging signature is not merely cohort-specific. Replication across cohorts remains essential[1][3][4][5].
