Entosis: A Cell-in-Cell Death Process

Entosis is a nonphagocytic cell-in-cell process in which one living cell actively enters another and may subsequently die inside the host cell. It was defined in matrix-detached epithelial cells as a nonapoptotic route that explains cell-in-cell structures long observed in human cancers. Unlike professional phagocytosis, the internalized cell participates in its own uptake and remains alive initially; it can undergo lysosomal degradation or be released. Entosis therefore combines cell invasion, competition, and death without fitting a simple execution pathway confined to one cell. Its importance has grown as tumor biology has shifted from viewing cell-in-cell structures as static morphology to testing how mechanical heterogeneity and nutrient stress select winners and losers within a cancer population[1][3][4].

The process begins with adherens-junction engagement and polarized actomyosin contraction. RhoA-controlled contractility in the entering cell generates force, while differences in deformability help determine which cell becomes the internalized loser and which becomes the host winner. Activated KRAS and RAC signaling can lower contractile myosin and confer winner behavior in heterogeneous populations. After entry, the host surrounds the inner cell with a single-membrane entotic vacuole. LC3 lipidation machinery, including ATG5, ATG7, and VPS34, targets that membrane and promotes lysosome fusion without requiring the upstream ULK complex used for canonical autophagosome formation. Acidification and lysosomal enzymes then kill and digest many inner cells. Glucose withdrawal activates AMPK in mechanically stiff losers and can drive entosis that transfers nutrients to proliferating winners[2][3][4].

Cancer provides the main application context, but entosis can produce opposing outcomes. Elimination of detached or mechanically disadvantaged cells may suppress tumor growth, whereas nutrient recovery and clonal competition may help aggressive winners survive metabolic scarcity. TRAIL signaling induces entosis in colorectal cancer cells through DR4 and DR5, with caspase-8 acting differently from its catalytic execution role in apoptosis; cell-in-cell structures at invasive tumor fronts were associated with adverse outcome in the reported cohort. These findings support cell-in-cell morphology as a research readout and nominate adhesion, contractility, lysosomal processing, metabolic sensing, and death-receptor signaling for intervention. No entosis-directed therapy is established, however, and blocking inner-cell killing could release viable cancer cells rather than eliminate the process[3][4][5].

The central gap is causal interpretation in intact tumors. A fixed section cannot establish whether an inner cell entered alive, whether it will die, or whether the structure arose through division or phagocytosis. Studies need longitudinal imaging, lineage tracing, mechanical measurements, lysosomal-flux assays, and perturbations that separate cell entry from vacuolar killing. Parallel assays should quantify host-cell survival and inner-cell fate under the same perturbation. Researchers must also determine which genotype becomes winner, whether host cells gain durable metabolic or proliferative advantages, and how immune cells respond to the products of digestion. Patient work should pair standardized cell-in-cell scoring with spatial molecular profiling and outcome analysis across tumor types. Selective manipulation may eventually exploit entosis to remove vulnerable cells or prevent winner-cell enrichment, but translation requires evidence that changing this process improves disease without disrupting epithelial architecture, autophagy-related membrane handling, or immune clearance[1][2][3][4][5].
