Metabolic Clusters, CVD Risk, and Gut Microbiota

Metabolic clusters provide a framework for separating heterogeneous cardiometabolic phenotypes according to patterns of glucose, lipid, renal, metabolomic, and microbiome-related variables. A recent cross-sectional study in individuals at high risk for diabetes identified three metabolic clusters characterized by high glucose or blood urea nitrogen, a relatively healthy metabolic profile, and lipid dysregulation, respectively; high microbial age was associated with elevated low-density lipoprotein and triglycerides and higher predicted cardiovascular disease (CVD) risk. Broader multi-omics studies have strengthened interest in this concept by showing that gut microbiome and metabolome alterations span the continuum from dysmetabolism to clinically manifest ischemic heart disease. In 1,241 participants, approximately 75% of microbiome and metabolome features distinguishing ischemic heart disease from health were already present in individuals with obesity or type 2 diabetes, suggesting that cardiometabolic microbial and metabolic shifts can precede overt cardiovascular disease[1][2].

Mechanistically, gut microbiota connect metabolic phenotypes with cardiovascular risk through microbial metabolism, host metabolism, immune signaling, intestinal barrier function, and circulating metabolites. Population-scale metagenomic analysis linked microbial pathways to plasma metabolites, lipoprotein subclasses, atherosclerosis, liver fat, inflammation, and a metabolic CVD risk score; 48 microbial pathways were associated with CVD risk largely independently of diet and inflammation. In atherosclerotic cardiovascular disease, metagenomic profiling showed enrichment of Enterobacteriaceae and Streptococcus species and altered microbial capacity for metabolism or transport of molecules relevant to cardiovascular health. Mechanistic work summarized in Circulation Research identifies trimethylamine N-oxide, phenylacetylglutamine, short-chain fatty acids, and secondary bile acids as major microbiota-related pathways; phenylacetylglutamine can promote adverse cardiovascular phenotypes through adrenergic receptors, while other microbiome-dependent metabolites interact with toll-like receptor and flavin monooxygenase signaling.

Gut microbiota may also influence cardiometabolic disease through nutrient and energy availability, immune activation, gut-barrier integrity, and systemic microbe-derived signaling molecules. These relationships create several applications for CVD risk stratification, disease biology, and therapeutic discovery. Acute coronary syndrome patients show distinct serum metabolomic and gut microbial signatures, including depletion of a Clostridiaceae species associated with circulating metabolites previously linked to coronary artery disease risk; person-specific metabolic deviations also correlate with clinical parameters and cardiovascular outcomes. Across the cardiometabolic disease spectrum, combined microbiome and metabolome signatures can distinguish ischemic heart disease from healthy or metabolically matched individuals, supporting investigation of multi-omic biomarkers for earlier risk characterization[3][4][5][6][7].

Diet represents another modifiable component: gut bacterial clusters or enterotypes have been correlated with cardiometabolic disease risk, while short-chain fatty acids, secondary bile acids, and trimethylamine link microbial metabolism with host phenotypes. Mediterranean and vegetarian dietary patterns have shown favorable associations with both gut microbiome features and cardiometabolic risk, supporting research on microbiome-mediated nutritional interventions. Important limitations remain before metabolic clusters or microbiome signatures can become reliable clinical tools. Cardiometabolic microbiome studies are vulnerable to confounding by medication, lifestyle, co-morbid disease, diet, age, sex, and other host characteristics, and associations between microbial composition and CVD do not by themselves establish causality[2][8][9].

Cardiometabolic effects are also heterogeneous: microbial influences may vary with host background, and blood and fecal measurements do not necessarily capture the same biologically relevant processes. Future work therefore needs longitudinal validation, mechanistic testing, standardized multi-omics methods, and replication across diverse populations before microbiome-derived clusters can support personalized prevention or treatment. The strongest translational opportunity is to integrate host metabolic phenotypes, microbial functions, circulating metabolites, and established CVD risk markers rather than relying on taxonomy alone, while evaluating microbiome-targeted diagnosis and treatment strategies in rigorously designed clinical studies[2][3][5][6][7].
