Cancer Stemness: Hallmarks And Plasticity

Cancer stemness describes a tumor-cell state marked by self-renewal, tumor initiation, differentiation capacity, therapeutic resistance, and plastic transitions between stem-like and non-stem-like phenotypes. The field began with evidence that human acute myeloid leukemia is organized as a hierarchy originating from primitive hematopoietic cells, then expanded to solid tumors when CD44+CD24−/lowLineage− breast cancer cells were prospectively identified as tumorigenic cells. Current research reframes cancer stemness less as a fixed rare population and more as a dynamic cell state shaped by tumor hierarchy, niche signals, epigenetic plasticity, and treatment pressure[1][2][3][4].

Mechanistically, cancer stemness depends on developmental signaling, epithelial-mesenchymal plasticity, microenvironmental control, and stress adaptation. EMT generates cells with stem-cell properties, high Wnt activity defines colon cancer stem cells under microenvironmental regulation, and Notch, Hedgehog, and Wnt pathways regulate stem-cell replication, survival, and differentiation. These mechanisms explain why stemness intersects with metastasis, immune evasion, quiescence, drug efflux, DNA damage repair, anti-apoptotic signaling, and metabolic rewiring rather than operating through one universal biomarker[5][6][7][8].

Disease applications focus on relapse prevention, metastasis biology, treatment resistance, biomarker discovery, and drug development. Cancer stemness provides a framework for studying why residual tumor cells survive chemotherapy, radiotherapy, targeted therapy, and immunotherapy, then regenerate heterogeneous disease. Therapeutic strategies include inhibiting Wnt, Notch, and Hedgehog signaling, disrupting niche support, forcing differentiation, targeting surface markers, and combining stemness-directed agents with standard therapy to reduce resistant minimal residual disease[3][7][8].

The major unresolved challenge is that cancer stemness is heterogeneous, plastic, and context-dependent, so marker-defined cancer stem cells do not always represent stable therapeutic targets. Future research should use functional assays, lineage tracing, single-cell profiling, spatial niche mapping, patient-derived models, and treatment-response studies to distinguish causal stemness drivers from correlated markers. The most realistic clinical prospect is integrated therapy that blocks plastic state transitions, disables stemness-maintaining pathways, and prevents relapse without assuming that every tumor follows a rigid cancer stem cell hierarchy[4][6][8][9].
