Fatty Liver Disease, Metabolic Disorders, and Bone Density

Metabolic dysfunction-associated steatotic liver disease (MASLD), historically studied as nonalcoholic fatty liver disease (NAFLD), is a metabolic liver disorder characterized by hepatic fat accumulation and close links with obesity, insulin resistance, dyslipidemia, metabolic syndrome, and type 2 diabetes; current work increasingly treats it as a multisystem disorder because skeletal complications can accompany liver disease. Research on the liver-bone relationship developed from evidence that obesity, diabetes, NAFLD, and osteoporosis share endocrine and inflammatory networks involving adipose tissue, liver, and bone. Pediatric work then showed that obese children with NAFLD had lower lumbar-spine bone mineral density (BMD) than matched obese controls, while later adult meta-analysis linked NAFLD with lower BMD and higher risks of osteoporosis and osteoporotic fracture. The field therefore has strong life-science relevance because fatty liver disease, metabolic disorders, bone remodeling, osteoporosis, and fragility fractures intersect in a common metabolic context[1][2][3].

Mechanistically, bone loss in MASLD reflects disturbed coupling between osteoblast-mediated bone formation and osteoclast-mediated bone resorption. In MASLD, insulin resistance can reduce anabolic insulin-receptor signaling in osteoblasts, while proinflammatory cytokines can enhance RANK/RANKL signaling in osteoclasts, shifting remodeling toward accelerated bone loss. Broader liver-bone-adipose crosstalk involves osteopontin, osteocalcin, osteoprotegerin, fetuin-A, leptin, tumor necrosis factor-α, and adiponectin, while liver-derived signals including IGF-1 and FGF21 have also been implicated in bone homeostasis. Experimental work on hepatic osteodystrophy further showed that restoring liver-derived IGF-1 improved cortical bone acquisition, microarchitecture, and mechanical properties in a liver-specific growth-hormone-receptor knockout model, but did not restore trabecular bone volume, indicating that liver-to-bone signaling is biologically heterogeneous across skeletal compartments[1][4][5].

Disease-oriented studies support bone-health assessment as an important research and clinical application in metabolic liver disease. In adults, a systematic review and meta-analysis found NAFLD associated with decreased BMD and increased risks of osteoporosis and osteoporotic fractures; another meta-analysis likewise found higher prevalence and risk of osteoporosis or osteoporotic fracture in NAFLD. Significant liver fibrosis has also been independently associated with low BMD at the femoral neck and total hip, suggesting that fibrosis severity may identify a subgroup with greater skeletal vulnerability. Mendelian-randomization research has extended this work toward causal inference: one study identified a causal relationship between NAFLD and femoral-neck BMD or osteoporosis in replication analyses, while also showing that adjustment for body mass index eliminated several fracture-related associations. These findings position the liver-bone axis as a target for risk stratification, biomarker research, and studies of interventions that jointly address metabolic dysfunction and skeletal deterioration[3][6][7][8].

Major gaps remain because epidemiologic results are heterogeneous across age, sex, ethnicity, body mass index, skeletal site, fibrosis status, and study design, and adult MASLD data can show apparently conflicting relationships between BMD and fracture risk. The available literature also indicates that BMD alone may not capture deterioration in bone architecture, while causal pathways connecting hepatic steatosis, insulin resistance, inflammation, hepatokines, adipokines, and bone remodeling remain incompletely resolved. Future studies should therefore use prospective designs, standardized MASLD and skeletal phenotyping, fracture outcomes, bone-turnover markers, and mechanistic measurements that distinguish cortical from trabecular bone. Clarifying whether liver-directed or metabolic therapies can improve bone quality, and whether bone-active therapies influence liver disease, could translate liver-bone biology into integrated prevention and treatment strategies for MASLD, osteoporosis, and metabolic disease[4][6][8][9].
