Sleep Deprivation: Molecular Mechanisms and Biological Consequences

 

As humans, we spend nearly one-third of our lives asleep, yet the precise molecular mechanisms and functions of sleep remain incompletely understood. Sleep plays a fundamental role in various biological processes such as the regulation of immune system, cognitive function as well as metabolism. Adults generally requires at least 7 hour of sleep per night for optimal health, but modern lifestyles have made sleep deprivation (SD) increasingly prevalent worldwide [1].

 

Animal studies show that IL-1, TNF-α, and PGD2 are involved in the physiologic and homeostatic non-rapid eye movement (NREM) sleep regulation. Therefore, the inhibition of these targets leads to decreased NREM sleep amount, whereas their administration suppresses rapid eye movement (REM) sleep and enhances NREM sleep intensity. With these cytokines, the immune system signals to the brain and interact with other factors (e.g. neurotransmitters, neuropeptides, nucleosides, melatonin, and the HPA axis), highlighting the bidirectional crosstalk between the immune system and sleep regulation [1].

 

SD triggers a wide range of cellular and physiological disturbances. Evidence from numerous animal models suggests that extended SD can result in premature mortality via the accumulation of reactive oxygen species (ROS) in the gut. SD leads to consequent health issues including chronic inflammation, increased cytokines production (e.g. IL-1 and TNF) and susceptibility to infection. SD can be broadly categorized as acute or chronic: acute SD refers to reduced or no sleep that lasts for 1 to 2 days, whereas chronic SD is characterized by excessive daytime sleepiness due to routinely reduced sleeping for over 3 months. A study also pointed out that most proinflammatory cytokines were upregulated as SD progressed, where IL-6 and IL-17A are the most notable SD-driven proinflammatory cytokines. In addition, CXCL1 and CXCL2 were also significantly elevated after SD [1] [2].

 

Both epidemiological and experimental data imply that SD is associated with risk of cardiovascular (e.g. hypertension, coronary artery disease) and metabolic (e.g. type 2 diabetes, obesity) diseases. Emerging evidence also connects chronic SD to accelerated aging, DNA damage accumulation, immunosenescence, and elevated risk for neurodegenerative diseases such as Alzheimer's disease [3] [4].

 

Together, these findings reveal sleep deprivation as a factor that disrupts immune balance, redox homeostasis, and systemic physiological resilience, underscoring the need to understand its molecular drivers and long-term health consequences.
