Mechanisms Linking Autoimmunity to Cardiovascular Disease

Autoimmunity increases cardiovascular risk through immune mechanisms that overlap with, but are not explained by, hypertension, dyslipidemia, and other conventional risk factors. Systemic lupus erythematosus, rheumatoid arthritis, antiphospholipid syndrome, and related diseases can accelerate atherosclerosis, raise blood pressure, promote thrombosis, and directly injure cardiac structures. Research has evolved from viewing vascular disease as a passive complication of systemic inflammation to defining shared leukocyte, cytokine, autoantibody, and tolerance pathways. Aberrant immune activation sustains endothelial dysfunction and defective resolution, while loss of tolerance to self-antigens can reproduce maladaptive features of atherosclerotic immunity. This mechanistic convergence makes autoimmune disease activity relevant to cardiovascular risk stratification and identifies immunomodulation as a potential complement to conventional prevention The cardiovascular phenotype can therefore arise from accelerated plaque biology, renal and vascular pressure regulation, acute thrombosis, or direct immune injury to cardiac tissue[1][2].

Modified lipoproteins provide one bridge between innate and adaptive immunity. Oxidized low-density lipoprotein triggers endothelial dysfunction and pro-inflammatory cytokine production, forms complexes with β2-glycoprotein I, and can activate endogenous sensors, inflammasome signaling, and autoreactive responses that intensify plaque lipid accumulation. Antiphospholipid syndrome supplies a second model in which autoantibodies couple inflammation to coagulation. Antiphospholipid antibodies drive monocytes toward cytokine- and tissue-factor-producing phenotypes through NF-κB, mTOR, and NLRP3 pathways; they also promote neutrophil extracellular traps, platelet activation, endothelial adhesion, and platelet-leukocyte interactions. Clinical applications span prevention, disease monitoring, and mechanism-directed therapy. Cardiovascular assessment should accompany autoimmune care because chronic inflammation can accelerate ischemic disease and immune complexes or inflammatory infiltrates can damage myocardium, valves, pericardium, and endocardium. Disease-activity biomarkers may identify patients who need intensified lipid, blood-pressure, and thrombosis management, but they must be tested against cardiovascular outcomes. Experimental strategies include inhibiting NLRP3, mTOR, tissue factor, neutrophil extracellular traps, or platelet adhesion pathways. Translation remains limited by heterogeneity among autoimmune diagnoses, treatments, vascular phenotypes, and stages of disease. Immunosuppressive drugs can reduce inflammatory activity yet also change infection, metabolism, thrombosis, and traditional cardiovascular risks, so observational associations cannot establish cardiovascular benefit. Future trials should define the causal immune circuit, separate plaque inflammation from systemic disease activity, and stratify by autoantibody profile, renal involvement, sex, blood pressure, and existing atherosclerosis. Longitudinal studies should determine whether biomarkers predict events independently of conventional factors and whether targeted immune treatment adds benefit to lipid lowering and antithrombotic care. Antigen-specific cellular therapies require rigorous evaluation of durability, off-target immune effects, manufacturability, and safety. The goal is not blanket immune suppression, but precise interruption of the autoimmune mechanism that is active in a particular cardiovascular lesion Prospective studies should compare cardiovascular events during controlled and uncontrolled autoimmune disease while accounting for immunosuppressive exposure, infection, renal function, and traditional prevention[1][3][5][6].
