PNS-Tumor-Immune Interplay in Cancer

The peripheral nervous system-tumor-immune axis is the reciprocal network linking autonomic and sensory fibers with malignant, stromal, vascular, and immune cells. Tumors can induce neurite growth and remodel local nerve activity, while neurotransmitters and neuropeptides alter cancer growth, blood supply, inflammation, immune-cell recruitment, and exhaustion. Immune cells can in turn sensitize or suppress sensory neurons, contributing to pain and additional mediator release. This framework extends the tumor microenvironment beyond cancer and immune cells to include a wired regulatory system with rapid local output and systemic connections. Research interest has accelerated because the same neural pathway may affect progression, symptoms, and response to immunotherapy[1][2].

Autonomic and sensory branches produce distinct mechanisms. In prostate cancer models, sympathetic fibers supported early tumor development through stromal adrenergic receptors, while parasympathetic cholinergic fibers promoted dissemination through muscarinic signaling. Adrenergic nerves also activated endothelial beta-receptor signaling and an angio-metabolic switch that supported tumor vascular growth. Sensory neurons can regulate immunity directly: in a melanoma model, tumor-associated nociceptors released calcitonin gene-related peptide, which acted through RAMP1 on cytotoxic T cells to increase exhaustion. Genetic removal or local silencing of nociceptors and RAMP1 antagonism reduced immune exhaustion and tumor growth in mice. These studies show that “innervation” is not one mechanism; fiber identity, transmitter, receptor-bearing cell, and disease stage determine the biological output[1][3][4][5].

Therapeutic strategies can interrupt neural recruitment, transmitter release, receptor signaling, or downstream immune and vascular responses. Candidates include local neural silencing, adrenergic or cholinergic antagonism, blockade of neurotrophic signals, and inhibition of the CGRP-RAMP1 pathway. Some targets already have approved neurological or cardiovascular drugs, but repurposing requires cancer-specific dosing and evidence. Systemic blockade may affect blood pressure, airway tone, pain, cognition, and host defense, whereas denervation can damage normal organ function. Biomarker-guided trials should identify the dominant fiber and responding cell before treatment and should test combinations with checkpoint blockade, antiangiogenic therapy, or conventional cancer treatment. Most causal evidence remains preclinical, so therapeutic claims must stay tumor- and model-specific[1][2][3][4][5].

Critical gaps include human circuit mapping, temporal direction, and standardized measurement. Nerve density on a fixed section does not reveal firing, transmitter release, origin, or connectivity, and pain is not a reliable surrogate for biological activity. Spatial transcriptomics should be combined with multiplex imaging, neural tracing, metabolite measurements, electrophysiology, and cell-specific receptor perturbation. Longitudinal samples must determine whether nerves initiate a permissive niche or expand after tumor remodeling. Comparative studies should analyze tumor core, invasive margin, adjacent tissue, draining lymph node, and metastatic sites because neural and immune interactions may differ across these compartments. Clinical studies should record stress, analgesics, psychiatric and cardiovascular drugs, and autonomic disease as prespecified covariates. Trial designs also need pharmacodynamic evidence that a neural intervention reaches the intended tumor circuit without broadly suppressing protective sensation or autonomic homeostasis. The most credible target will connect a defined peripheral fiber to a molecular signal, a specific tumor or immune cell, a measurable patient biomarker, and an outcome that improves beyond standard therapy without unacceptable neurological toxicity[1][2][3][4][5].
