Ferroptosis and Cardiovascular Disease

Ferroptosis is an iron-dependent form of regulated cell death driven by lethal accumulation of phospholipid hydroperoxides. It differs from apoptosis and necrosis through its dependence on iron, polyunsaturated membrane lipids, amino-acid and glutathione metabolism, and specialized antioxidant systems. Cardiovascular interest has expanded rapidly because iron imbalance and lipid oxidation occur in atherosclerosis, myocardial ischemia-reperfusion injury, drug-induced heart failure, sepsis-related cardiomyopathy, arrhythmia, and diabetic cardiomyopathy. The heart requires tightly controlled iron for metabolism but is vulnerable to oxidative damage when uptake, storage, export, or mitochondrial use is disturbed. Ferroptosis therefore connects metabolic homeostasis, redox defense, membrane composition, and cell loss within one druggable framework Ferroptosis research is particularly relevant to myocardium because high oxygen use, abundant mitochondria, iron-dependent metabolism, and polyunsaturated membranes create a vulnerable biochemical environment[1][2].

Execution occurs when iron-catalyzed oxidation overwhelms defenses that remove lipid peroxides. The cystine-glutamate antiporter system Xc− supplies cystine for glutathione synthesis, and glutathione peroxidase 4 reduces membrane hydroperoxides; loss of SLC7A11, glutathione, or GPX4 activity sensitizes cells to death. In prolonged myocardial reperfusion, ALOX15 generates 15-hydroperoxyeicosatetraenoic acid, which promotes ubiquitin-dependent degradation of PPARGC1A, impairs mitochondrial biogenesis, and aggravates cardiomyocyte ferroptosis. A separate pathway links DNA sensing to antioxidant loss: cGAS-STING signaling targets GPX4 for autophagic degradation, forming a positive feedback circuit that increases oxidative injury. Genetic or pharmacological interruption of either axis improves experimental cardiac recovery ALOX15 metabolites and STING-dependent GPX4 degradation identify separate routes to the same lethal lipid-peroxide threshold, suggesting that upstream context should guide inhibitor selection[1][3][4].

In atherosclerotic plaques, iron deposition, oxidized lipids, and excessive death create conditions compatible with ferroptosis in endothelium, macrophages, smooth-muscle cells, dendritic cells, and lymphocytes. The effect is likely stage- and cell-dependent because loss of endothelial integrity, macrophage dysfunction, and smooth-muscle depletion can influence lesion growth and stability differently. Candidate therapies include iron handling, radical-trapping antioxidants, lipid-remodeling enzymes, GPX4 preservation, system Xc− support, ALOX15 inhibition, STING blockade, repurposed drugs, and targeted delivery. However, compounds that reduce lipid oxidation may affect multiple processes, so protection cannot be assigned to ferroptosis without genetic or biochemical confirmation. Current cardiovascular evidence is strongest in experimental models rather than outcome trials Plaque interventions must also consider that death of macrophages, endothelial cells, and smooth-muscle cells can produce different consequences for inflammation, barrier integrity, and cap stability[2][4][5][6].

The principal gap is diagnostic rigor. Iron accumulation, reactive oxygen species, altered GPX4, and lipid peroxidation are not individually specific for ferroptotic death, and different death programs can coexist after reperfusion or within plaques. Future work should establish temporal sequence, cell identity, membrane-lipid products, rescue by mechanistically distinct inhibitors, and dependence on essential ferroptosis regulators. Human studies need validated tissue or circulating markers linked to disease stage and clinical outcome. Therapy must also preserve physiological iron use and avoid impairing antimicrobial defense or beneficial lipid signaling. Cell-targeted delivery may be necessary because suppressing ferroptosis in one plaque population could differ from protecting cardiomyocytes during reperfusion. Translation should proceed through mechanism-defined cardiovascular subgroups, not a universal assumption that all oxidative cell death is ferroptosis Clinical development needs assays that distinguish ferroptosis suppression from general antioxidant effects and show preservation of viable cardiovascular tissue without promoting unwanted cell survival elsewhere[1][5][6][7].
