Ferroptosis Route to Tumor Immune Remodeling

Ferroptosis route to tumor immune remodeling refers to the integration of iron-dependent lipid peroxidation with antitumor immunity, whereby ferroptotic signaling reshapes the tumor microenvironment and influences immune-cell function. Ferroptosis was initially defined as a regulated form of cell death driven by lipid peroxidation and oxidative membrane damage, and subsequent cancer studies established a direct connection between ferroptosis and immunotherapy efficacy. A major advance was the demonstration that immunotherapy-activated CD8⁺ T cells enhance ferroptosis-specific lipid peroxidation in tumor cells and that ferroptosis contributes to the antitumor efficacy of immune checkpoint blockade. These findings positioned ferroptosis as a mechanistic component of the cancer-immunity cycle rather than solely a tumor-cell intrinsic death program. The topic has since become a major focus in cancer biology, tumor microenvironment research, and cancer immunotherapy because ferroptosis links metabolic regulation, redox homeostasis, immune activation, and therapeutic response[1][2][3].
The central mechanism of tumor immune remodeling through ferroptosis involves IFN-γ released by activated CD8⁺ T cells. IFN-γ suppresses the expression of SLC3A2 and SLC7A11, the two subunits of system Xc⁻, thereby reducing cystine uptake, promoting lipid peroxidation, and sensitizing tumor cells to ferroptosis. This regulation is mediated through the JAK1-STAT1-IRF1 signaling axis and alters cellular antioxidant capacity. Additional studies showed that IFN-γ can cooperate with arachidonic acid metabolism and ACSL4-dependent pathways to amplify ferroptotic susceptibility. Ferroptosis also feeds back to immune regulation because ferroptotic tumor cells can increase antigen availability and strengthen antitumor immune responses, whereas ferroptosis occurring in immune cells may impair immune surveillance. Consequently, ferroptosis functions as a bidirectional regulator connecting tumor-cell death and immune-cell fitness within the tumor microenvironment[1][2][4][5][6].
Current applications of ferroptosis research include combination strategies with immune checkpoint blockade, identification of ferroptosis biomarkers, development of ferroptosis-inducing drugs, and investigation of tumor microenvironment remodeling. Preclinical studies demonstrated that cyst(e)inase-mediated depletion of cystine or cysteine synergizes with checkpoint blockade and enhances T-cell-mediated antitumor immunity. Emerging evidence further indicates that modulation of GPX4, ACSL4, SLC7A11, and related lipid-metabolism pathways may improve therapeutic responses and influence immune-cell composition within tumors. However, major gaps remain regarding cell-type-specific ferroptosis responses, determinants of sensitivity across tumor types, and the balance between promoting tumor-cell ferroptosis while preventing ferroptotic dysfunction of antitumor lymphocytes. Future studies should integrate ferroptosis biomarkers, immune profiling, metabolic analyses, and rational combination therapies to define how ferroptosis can be exploited for durable tumor immune remodeling and precision cancer immunotherapy[1][4][5][6][7].
