SIRT3 and Cardiovascular Diseases

Sirtuin 3 (SIRT3) is a mitochondrial NAD+-dependent deacetylase that adjusts enzyme activity through lysine deacetylation. It coordinates oxidative metabolism, ATP production, antioxidant defense, mitochondrial dynamics, calcium handling, and stress resistance in tissues with high energetic demand. The heart and vasculature depend on these processes, and reduced SIRT3 expression or activity is associated experimentally with hypertrophy, fibrosis, heart failure, endothelial dysfunction, hypertension, and vascular remodeling. The research trajectory has moved from cataloguing mitochondrial acetylation to defining individual SIRT3 substrates and testing small-molecule activation. This target is attractive because it can correct a regulatory layer shared by metabolism, redox balance, and organelle quality rather than one downstream injury signal SIRT3 is therefore positioned as a mitochondrial control node whose loss can translate nutrient and redox imbalance into several forms of cardiovascular remodeling[1][2].

SIRT3 activation can restrain several remodeling pathways. The small molecule 2-APQC requires SIRT3 to reduce experimental hypertrophy and fibrosis; it inhibits mTOR-p70S6K, JNK, and TGF-β-SMAD3 signaling, activates the SIRT3-PYCR1 axis and mitochondrial proline metabolism, reduces ROS-p38 MAPK signaling, and supports AMPK-Parkin-dependent protection. SIRT3 deficiency also increases p53 acetylation, lowers GPX4, promotes ferroptosis in myofibroblasts, and worsens cardiac fibrosis. In pressure overload, reduced SIRT3 makes cardiomyocytes sensitive to copper accumulation and cuproptosis, whereas cardiomyocyte SIRT3 overexpression attenuates pathological remodeling. These results connect deacetylation to multiple death and repair programs but remain predominantly preclinical Its substrates connect deacetylation to antioxidant defense, proline metabolism, calcium exchange, histone acetylation, vascular phenotype, and regulated cell death rather than one linear pathway[1][2][3][4].

Vascular effects add clinically relevant substrates. In human hypertension and experimental angiotensin-II exposure, reduced SIRT3 and increased GCN5L1 raise cyclophilin-D acetylation at K166, increase mitochondrial superoxide, impair endothelial NO, shift metabolism toward glycolysis, and promote hypertension. In vascular smooth muscle, SIRT3 loss causes mitochondrial acetyl-CoA accumulation, increased H3K27 acetylation at the KLF4 promoter, phenotypic switching, neointima formation, and aneurysmal remodeling; restoring SIRT3 or limiting cytosolic acetyl-CoA transport is protective. In hypertrophic cardiomyocytes, SIRT3 deacetylates ATAD3A and regulates ER-mitochondria contacts, IP3R1-GRP75-VDAC1 signaling, mitochondrial calcium loading, and bioenergetics Disease-specific activation may be required because the most relevant downstream substrate in hypertensive endothelium differs from that in fibroblasts, cardiomyocytes, or smooth-muscle cells[3][4][5][6].

Translation faces questions of specificity, exposure, and causal hierarchy. Compounds labeled as SIRT3 activators must show direct biochemical engagement, mitochondrial access, and loss of effect when SIRT3 is absent. Broad deacetylation could alter many substrates, and benefit may depend on cell type, disease stage, NAD+ availability, or the dominant injury program. Human studies should measure mitochondrial acetylation signatures and connect them to cardiac or vascular function before selecting patients. Trials must distinguish chronic prevention from acute rescue and monitor liver, kidney, metabolism, and interactions with other sirtuins. Substrate-specific approaches may offer greater precision than global activation.
