Inflammatory Pathways Mediate Iron Metabolism and Lipid Peroxidation

Inflammation reorganizes iron distribution as part of host defense, but persistent iron sequestration can impair erythropoiesis and increase oxidative stress. Cytokines stimulate the hepatic hormone hepcidin, which binds the iron exporter ferroportin and reduces iron release from enterocytes and macrophages. IL-6 was shown to induce hepcidin during inflammation and produce hypoferremia, establishing a direct cytokine-iron-regulatory axis. This response can restrict extracellular iron available to microbes while moving iron into storage compartments. Chronic activation contributes to anemia of inflammation and creates tissue-specific iron loading, especially when macrophage recycling, ferritin storage, erythropoietic demand, or renal function is altered. Current research connects this systemic pathway to intracellular lipid peroxidation and ferroptosis, where iron availability changes the probability that oxidized phospholipids become lethal[1][2].

Lipid peroxidation becomes ferroptotic when iron-dependent radical reactions overwhelm membrane repair. Polyunsaturated fatty acids must be incorporated into phospholipids, oxidant pressure must initiate and propagate damage, and antioxidant systems must fail. System xc− supports cystine uptake and glutathione synthesis; GPX4 uses glutathione to reduce phospholipid hydroperoxides, while parallel radical-trapping pathways provide additional defense. Inflammatory cells can alter this network through reactive oxygen and nitrogen species, cytokines, iron uptake and export, and oxidized-lipid production. Conversely, ferroptotic cells release oxidized lipids and danger signals that recruit or reprogram immune cells. Iron sequestration is therefore not uniformly protective or harmful: it may limit pathogens systemically while increasing the labile iron burden of a particular macrophage or parenchymal cell[3][4][5].

The combined axis affects infection, cancer, atherosclerosis, ischemia-reperfusion injury, kidney and liver disease, and neurodegeneration. Hepcidin or ferroportin modulation can address selected forms of inflammatory anemia, whereas ferroptosis inhibitors may protect tissues exposed to iron-catalyzed lipid damage. Cancer therapy pursues the opposite outcome by blocking cystine transport or GPX4 defense to induce ferroptosis in metabolically susceptible tumor cells. Combination strategies must consider immunity: reducing IL-6-hepcidin signaling may restore circulating iron but could change pathogen access, and inducing tumor ferroptosis may produce either productive immunity or tumor-supportive inflammation. Biomarkers should measure systemic iron handling, cell-specific labile iron, phospholipid oxidation, and execution-pathway rescue rather than treating ferritin elevation or lipid peroxidation as proof of one mechanism[1][3][4][5].

Important gaps lie between organismal and subcellular scales. Serum hepcidin, ferritin, and transferrin saturation do not reveal which cell contains redox-active iron, while common oxidation probes lack lipid and compartment specificity. Future studies should combine iron isotopes, spatial metal imaging, targeted lipidomics, single-cell inflammatory profiling, and genetic perturbation of ferroportin, GPX4, and lipid-remodeling enzymes. Time-resolved experiments must distinguish inflammation that precedes ferroptosis from inflammation caused by ferroptotic damage. Clinical trials need paired markers of iron redistribution and membrane oxidation, plus safety monitoring for infection, anemia, and collateral organ injury. Parallel microbial assays are also needed whenever treatment changes extracellular iron availability or macrophage iron handling. Selective delivery may allow tumor-cell ferroptosis or tissue protection without globally changing iron availability. A mechanistic map of cytokine source, iron destination, oxidized lipid species, and responding immune cell will be essential for rational intervention[1][2][3][4][5].
