Interplay Between Macrophages and Ferroptosis in Disease Therapy

Ferroptosis is an iron-dependent regulated cell death driven by unchecked phospholipid peroxidation, whereas macrophages are phagocytic immune cells that coordinate iron recycling, lipid metabolism, inflammation, and tissue repair. Their intersection is therefore intrinsic rather than incidental. Macrophages can alter the iron, cytokine, and redox environment that determines whether neighboring cells undergo ferroptosis; they can also become ferroptotic themselves or respond to molecules released by ferroptotic cells. Since ferroptosis was defined in 2012, research has moved from tumor-cell-autonomous metabolism toward multicellular control in cancer, infection, ischemic injury, and degenerative disease. Therapeutic interest stems from a double opportunity: induce ferroptosis in resistant malignant cells while shaping macrophage responses, or suppress ferroptotic damage and macrophage-driven inflammation in vulnerable normal tissue[1][2][3].

The core ferroptosis network couples iron availability and polyunsaturated phospholipids to antioxidant defense. System xc− supplies cystine for glutathione synthesis, GPX4 reduces phospholipid hydroperoxides, and failure of these defenses allows radical propagation across membranes. Macrophage polarization and activation alter iron sequestration or export, reactive-oxygen-species production, lipid handling, and cytokine release, thereby changing this threshold. Ferroptotic cells also communicate outward. Decorin released through secretory autophagy and lysosomal exocytosis binds AGER on macrophages and activates NF-κB-dependent proinflammatory cytokine production; disrupting this axis reduced ferroptosis-associated acute pancreatitis and limited a tumor-protective immune response in experimental models. These data identify a specific signal-receptor circuit but do not support treating all ferroptotic-cell products as uniformly immunostimulatory or antitumor[2][3][4].

Cancer illustrates the therapeutic tension. Ferroptosis in tumor cells can remove apoptosis-resistant clones and release danger signals, yet the resulting macrophage response may build a tumor-supportive inflammatory niche. In hepatocellular carcinoma, macrophages encountering ferroptotic cancer cells produced IL-1β through NF-κB and NLRP3-related signaling; recruited neutrophils remodeled sinusoidal vasculature, promoted metastasis, and reduced sorafenib efficacy. Depleting relevant immune cells or neutralizing IL-1β suppressed this network in vivo. A rational combination may therefore pair a ferroptosis inducer with blockade of a defined macrophage inflammatory output rather than globally suppressing innate immunity. Outside cancer, ferroptosis inhibitors may protect tissues where macrophage activation amplifies oxidative injury, while cell-selective induction could remove pathogenic macrophage subsets; both concepts require evidence that benefit arises from ferroptosis modulation rather than generic antioxidant or anti-inflammatory activity[2][3][5].

Major gaps concern cell identity, timing, and human relevance. Common measurements—lipid-oxidation dyes, malondialdehyde, iron content, ACSL4, or reduced GPX4—do not independently prove ferroptosis, and macrophage markers do not capture the continuous states present in diseased tissue. Studies should use cell-specific genetic perturbation, ferroptosis rescue, spatial mapping, lipidomics, and longitudinal profiling to separate ferroptotic macrophages from macrophages responding to ferroptotic neighbors. Drug development must determine whether to target system xc−, GPX4, iron handling, AGER, NLRP3, or downstream cytokines in each disease. Selective delivery is essential because systemic ferroptosis induction can damage immune and parenchymal cells, whereas broad inhibition may protect tumors. Human organoids, ex vivo tissues, and pharmacodynamic biomarkers should test whether the macrophage-ferroptosis circuits defined in mice persist in patients and identify the treatment window before inflammation becomes self-sustaining[1][2][3][4][5].
