Osteoporosis as a Metabolic Bone Disease

Osteoporosis is the most common metabolic bone disease and is characterized by reduced bone strength, fragility fractures, and substantial morbidity, mortality, and personal and economic burden. Modern clinical guidance emphasizes that the disease can remain subclinical until fracture and that recurrent fractures can drive disability and premature death. The research trajectory has moved from predominantly antiresorptive treatment toward mechanism-based intervention: antiresorptive drugs were the treatment of choice for decades, while advances in molecular biology and genetics produced a more detailed understanding of the bone-remodeling cycle and revealed new therapeutic targets. This progress has made osteoporosis a major focus of skeletal biology, aging research, fracture prevention, and translational drug discovery, yet a marked care gap persists because many high-risk patients are not screened, diagnosed, treated, or adherent to therapy[1][2].

At the biological level, osteoporosis reflects disruption of bone remodeling, a coupled process in which osteoclasts remove damaged bone and osteoblasts replace it. Osteoclast formation and function are regulated especially by receptor activator of NF-κB ligand (RANKL) and macrophage colony-stimulating factor (M-CSF), while osteoblasts, osteocytes, and immune cells contribute signals that control osteoclast differentiation, activity, and lifespan. Osteocytes provide an additional regulatory layer: they coordinate osteoblast and osteoclast function, express the bone-formation inhibitor sclerostin, and produce osteoprotegerin (OPG) and RANKL that regulate osteoclast generation and activity. The RANKL/OPG axis and canonical Wnt signaling therefore connect bone resorption with bone formation.

Hormonal imbalance can shift this system toward bone loss; estrogen promotes OPG, suppresses RANKL activity, activates Wnt/β-catenin signaling, and supports osteogenesis, whereas estrogen deficiency increases bone turnover and inflammatory cytokine production. Excess glucocorticoids can also increase the RANKL/OPG ratio, inhibit Wnt- and BMP-related osteogenic signaling, and promote osteoblast and osteocyte apoptosis. These mechanisms directly inform osteoporosis therapeutics. Bisphosphonates and the RANKL inhibitor denosumab are widely used antiresorptive agents that increase bone mineral density and reduce vertebral, nonvertebral, and hip fracture risk[2][3][4][5].

Denosumab neutralizes RANKL, thereby inhibiting osteoclast differentiation, activity, and survival; long-term evidence shows progressive BMD gains and sustained low vertebral fracture rates, but stopping treatment can cause rebound bone turnover, rapid BMD loss, and increased risk of multiple vertebral fractures, making transition to another anti-osteoporosis therapy important. Anabolic treatment targets bone formation: teriparatide has demonstrated superiority to risedronate for preventing vertebral and clinical fractures in women with vertebral fracture, while romosozumab binds sclerostin, increases bone formation, and decreases bone resorption. In a large randomized trial, romosozumab lowered vertebral fracture risk at 12 months and retained benefit after transition to denosumab. Current research gaps center on translating molecular precision into durable, safe, and accessible fracture prevention[6][7][8].

Antiresorptive agents do not rebuild bone architecture, and new biological agents can produce unexpected adverse effects because bone-remodeling cytokines and proteins may act in other tissues. Severe osteoporosis may therefore benefit from sequential strategies that begin with a bone-building drug and continue with an antiresorptive agent, while post-denosumab transition requires careful management to prevent rebound loss. Future work should refine anabolic-antiresorptive sequencing, clarify long-term safety of pathway-specific agents, identify approaches that preserve treatment gains after discontinuation, and close the persistent gap between effective pharmacology and real-world diagnosis, initiation, and adherence[1][2][6][7].
