PANoptosis: Crosstalk Between Pyroptosis, Apoptosis and Necroptosis

PANoptosis is a regulated inflammatory cell death pathway that integrates key features of pyroptosis, apoptosis, and necroptosis and cannot be fully explained by any individual pathway alone[1][2][3][4]. The concept emerged from studies demonstrating extensive molecular crosstalk among classical programmed cell death pathways during host responses to infection and inflammatory stimuli, leading to the recognition of PANoptosis as a distinct biological process regulated by multiprotein PANoptosome complexes[1][2][5][6]. Current research identifies PANoptosis as an important mechanism linking innate immunity, pathogen sensing, inflammation, tissue injury, and disease progression. The pathway has become a major research focus because it provides a framework for understanding how cells coordinate multiple death programs in response to pathogens, cytokines, damage-associated molecular patterns, and cellular stress signals[1][2][5][6][7].
Mechanistically, PANoptosis is driven by PANoptosome complexes that integrate components traditionally assigned to inflammasomes, apoptotic signaling complexes, and necrosomes[2][5][6][7]. Key molecular regulators include ZBP1, AIM2, RIPK1, RIPK3, ASC, caspase-1, caspase-8, gasdermins, and MLKL, which function in interconnected signaling networks rather than isolated pathways[2][5][6][7]. PANoptosome assembly enables coordinated activation of pyroptotic, apoptotic, and necroptotic execution mechanisms, producing inflammatory cell death responses that cannot be blocked by inhibition of a single pathway alone[1][2][3][5]. Experimental studies have identified multiple PANoptosome configurations, including ZBP1-PANoptosome, AIM2-PANoptosome, NLRP3-PANoptosome, NLRP12-PANoptosome, and RIPK1-PANoptosome complexes, highlighting stimulus-specific regulation of cell death outcomes[2][5][7]. These findings establish PANoptosis as a central signaling hub that coordinates caspase-dependent and RIPK-dependent cell death programs during infection, inflammation, and immune activation[2][5][6][7].
PANoptosis has been implicated in infectious diseases, inflammatory disorders, cancer, neurodegenerative conditions, and tissue injury models[1][3][4][7][8]. In cancer biology, PANoptosis has attracted attention because simultaneous activation of pyroptosis, apoptosis, and necroptosis may overcome resistance mechanisms that limit therapies targeting a single cell death pathway[3][8]. Research also indicates that PANoptosis contributes to host defense against bacterial, viral, and fungal pathogens through coordinated inflammatory cell death responses[2][6][7]. Despite rapid progress, important questions remain regarding PANoptosome composition in different tissues, context-specific pathway regulation, biomarkers of PANoptosis activation, and therapeutic strategies that selectively modulate PANoptosis without causing excessive inflammation. Future studies are expected to refine molecular definitions of PANoptosome complexes and evaluate PANoptosis-targeted interventions in inflammatory disease and cancer therapy[1][2][3][5][8].
